Spodick's sign: a helpful electrocardiographic clue to the diagnosis of acute pericarditis.

چکیده
Dear Editor, We read with great interest the work by Masek and Levis published in a recent issue of The Permanente Journal. They describe a classical electrocardiographic evolution of acute pericarditis. In the presented first electrocardiogram (ECG) of Stage I acute pericarditis, we would like to bring to the readers’ attention an interesting ECG finding: Spodick’s sign (Figure 1). This sign has been named after our University professor David Spodick, MD, a world-famous physician for his work on the pericardial diseases. It signifies to a downsloping TP segment in patients with acute pericarditis and is present in about 80% of the patients affected with acute pericarditis. The sign is often best visualized in lead II and lateral precordial leads. In addition, Spodick’s sign may also serve as an important distinguishing electrocardiographic tool between the acute pericarditis and acute coronary syndrome. PR-segment depression when used alone can sometimes be a masquerader, as it can be seen in both acute pericarditis and acute coronary syndrome. However, the presence of PR depression and Spodick’s sign is often a giveaway to the diagnosis. In Masek and Levis’s Figure 1 ECG (www. thepermanentejournal.org/issues/2013/ fall/5537-acute-pericarditis.html), TP-segment downsloping is clearly apparent especially in leads II, V4-6 with near complete resolution after treatment with nonsteroidal anti-inflammatory drugs (Masek and Levis’s Figure 2 [www.thepermanentejournal.org/issues/2013/ fall/5537-acute-pericarditis.html]). v

Regional Pericarditis or an Alternate Diagnosis?
We read the paper by Alhammouri and Omar published in a recent issue of the Case Reports in Medicine [1]. The authors have described a case of what appeared to be a case of localized pericarditis; however, we wonder if alternate diagnosis was considered in the differential diagnosis. The initial ECG illustration shows ST-elevation localized to the inferior leads with reciprocal ST-depressions i...
متن کاملConstrictive pericarditis masquerading as mitral stenosis.
The five patients presented were considered to have mitral stenosis because of a history of dyspnea on exertion or paroxysmal atrial fibrillation, the presence of a loud first heart sound with an "opening snap," a loud pulmonary component of the second heart sound, electrocardiographic evidence of right axis deviation and left atrial enlargement or radiological evidence of cardiomegaly, especia...
متن کاملRegional Pericarditis Mimicking Inferior Myocardial Infarction following Abdominal Surgery
Acute pericarditis is common but illusive, often mimicking acute coronary syndrome in its clinical and electrocardiographic presentation. Regional pericarditis, though rare, presents further challenge with a paucity of published diagnostic criteria. We present a case of postoperative regional pericarditis and discuss helpful electrocardiographic findings. A 66-year-old male with history of open...
متن کاملElectrocardiographic manifestations and differential diagnosis of acute pericarditis.
Acute pericarditis has many potential etiologies and typically presents as a sharp central chest pain that worsens with recumbency and is relieved by leaning forward. The pathognomonic physical finding of acute pericarditis is the pericardial friction rub, which is usually auscultated along the lower left sternal border. The electrocardiogram (ECG) is a useful, simple tool that may aid in the d...
متن کاملKussmaul's sign and rapid X‐Y descents in constrictive pericarditis
Right-sided heart failure is not uncommon. A careful jugular venous examination looking for the distinct Kussmaul's sign and rapid X-Y descents, which are highly indicative of constrictive pericarditis, is markedly helpful in guiding the correct diagnosis and proper management.
